ConnectCare is a non-profit agency which provides quality services in the privacy of your own home. Our home care services include skilled nursing care, medication assistance, speech, physical and occupational therapy, home health aide visits, assistance with personal cares, companionship, homemaking and meal preparation. In addition, hospice services also include respite care, music intervention, pet therapy, chaplain, social worker and volunteer visits, grief education and other bereavement services. We provide compassionate end of life care to clients and their families who are experiencing a progressive, terminal illness. The focus of hospice is to provide comfort care while maintaining dignity and continued quality of life when a cure is no longer possible.
