Horizons Health Care Center has been serving the community since 1972 and provides quality skilled nursing care and inpatient/outpatient rehabilitation. It was founded by a local physician for the local community. Horizons Health Care is an integral part of the Surface Creek community of Cory, Austin, Eckert & Cedaredge. We are located in the shadows of the Grand Mesa on nine acres in rural western slopes of Colorado. We care for individuals with chronic diseases and for those recovering from injury or illness. We have an 11-bed rehab wing with private and semi-private rooms dedicated to short term rehab residents with our goal to enable them to return to their home. We also have a 16 bed Memory Care Unit for the care of individuals with Alzheimer's and other dementia type illness who wander and/or need quiet structured environment. Our staff provides kind, compassionate care to our 'family' and we welcome families & friends to be involved with their loved one's care and activities.
